On May 19 2024, an A&E doctor collapsed from sudden cardiac arrest during a tennis game at his condominium. His fellow doctor friends quickly called for ambulance, performed CPR, and searched for an AED, but none were available in the building. They had to sprint to a nearby location, and by the time the AED arrived, he had been unresponsive for 15 minutes—far too long in a cardiac emergency. For one, you will need to ensure that your AED machine is placed in a visible and unobstructed location. From there, you can carry on with the specific fortnightly and monthly checks:

1. Fortnightly Checks

Once every two weeks, conduct a visual check. This means checking for any damage, missing parts, cracks, or chips on the case, signage, and device. 2. Monthly Checks

For one, as mentioned, both types of AED machines are used to treat sudden cardiac arrest by analysing the heart rhythm of the patient and delivering an electric shock to the heart to restore a normal rhythm. Designed to provide visual and audible prompts to guide the user through the defibrillation process, these modern life-saving devices can be utilised even by individuals with little to no medical training.

Differences Between Fully and Semi-Automatic AEDs

While both AED machines provide clear and concise instructions throughout the defibrillation process, the biggest difference between the two comes down to whether the shock will be delivered automatically or only when the shock button is pressed manually. Due to this singular distinguishing feature, there is a long list of differences to take into account when deciding on which defibrillator to choose:

1. Speed of Treatment

Since the rescuer is relied upon to press the shock button of the semi-automatic AED, extended periods of hesitation or even shock are factors that might hinder an individual from treating the patient promptly. This is where the fully-automatic AED has the upper hand due to its ability to administer the shock automatically when required, without any additional human involvement beyond the application of the electrode pads to the patient.

2. Control Over the Situation

From the above, it is clear to see that with a semi-automatic AED, responders have sole control over when to deliver a shock whereas with a fully automatic device, responders do not. This means that responders must be comfortable with relinquishing some control over SCA treatment for a fully automatic device to be used. On the same note, some users feel more comfortable with a semi-automatic AED because they feel like they have more control over the situation. But this increases the chances of user errors such as pressing the shock button at the wrong time or not pressing the button at all.

However, it is important to remember that both types of AED machines will only deliver a shock if one is needed — the user does not have to worry about accidentally shocking someone who does not need it.

3. Ease of Use

Both semi-automatic and fully automatic AED machines are designed to be easy to use, even for those with no medical training since clear and concise instructions are provided. That being said, some may find one type of AED easier to use than the other. For example, with a fully automatic AED, the extra step of ensuring that no one is touching the patient or holding their hands before administering the shock must be taken. Either way, it is crucial to read the instructions carefully and familiarise yourself with the device, or even go for a defibrillator training before using it in an emergency.

According to recent statistics, almost 3,000 people have a sudden cardiac arrest in Singapore every year. Cardiac arrest has also been the cause of more fatalities compared to cancer, stroke, or trauma. This indicates that it can happen to anyone anywhere, even in the workplace.

In reference to the updated Fire Safety Act 1993, all establishments in Singapore with Fire Safety Managers are required to have a Company Emergency Response Team (CERT). The CERT is composed of employees who will act as first responders to prevent any emergency from escalating, including sudden cardiac arrests.

Aside from basic first aid skills, your company emergency response team should undergo cardiopulmonary resuscitation (CPR) and automatic external defibrillators (AED) training in Singapore.

CPR is a technique that’s used in emergencies in which someone’s breathing or heartbeat has stopped. The technique can keep oxygen-rich blood flowing to the brain and other organs until emergency medical treatment can restore the person’s heart rhythm. An AED, on the other hand, is a medical device that’s used to analyse the heart rhythm and deliver an electric shock to victims of sudden cardiac arrest to restore the heart rhythm to normal. Typically, it is only used when someone is unconscious and not breathing normally following a cardiac arrest.

According to the European Resuscitation Council, when CPR is performed along with the use of AED within 3 to 5 minutes of collapse or before EMS arrives, the victim’s chances of survival increase by up to 75%. 3. It prevents the chances of brain death or other conditions

More than 2,500 Out-of-Hospital Cardiac Arrest (OHCA) occurs every year in Singapore, out of which only 5.3% of victims survive the deadly incidents⁽¹⁾. Even with Singapore’s advanced healthcare response and emergency rescue, the brain start dying soon after the victim collapsed and the heart’s stop beating. In order to save a cardiac arrest victim, the only possible rescue procedure as a lay-rescuer or bystander, is to apply effective early CPR AED resuscitation.

When is an AED used?

Before we explore the use of AED, we must understand the situation that calls for the need for an AED.

An AED can only be used in a cardiac arrest emergency. However, more than often, cardiac arrest is often misunderstood as heart attack.

A heart attack is NOT the same as sudden cardiac arrest. A heart attack happens as a result of blockage due to build-up of cholesterol or plaque, obstructing the blood flow to the heart. While sudden cardiac arrest (SCA) strikes when the heart stops beating due to electrical disturbance in your heart that disrupts its pumping action, stopping blood flow to the rest of the body. Unfortunately, a heart attack is known to develop into sudden cardiac arrest when left untreated.

Sudden cardiac arrest is easily identified by the symptoms of:

Sudden collapse
Loss of consciousness; and
No sign of pulse and breathing

When you spot these signs, immediately call for the local ambulance, get an AED and start chest compressions.

How can CPR AED help?

Automated External Defibrillator (AED) is a portable device that checks the heart rhythm of a suspected cardiac arrest emergency. If needed, an AED delivers the shock to revive the victim by restoring the heart rhythm to start pumping blood effectively.

However, AED is often not accessible at hand immediately to assess and deliver the shock. And worse, not all cardiac arrests are identified as shockable cases. Hence in some situation, your AED may not be as useful as you think. This is one of the reasons why there is a low percentage of survival for cardiac arrest victim.

Despite the continuous effort to promote CPR AED awareness, it is still not enough. Not only from the perspective on the number of AEDs installed, but also on the awareness of AED locations, and the knowledge to apply AED when needed.

Hence, even though you purchased an AED, but if your employees/family members do not have the confident or proper knowledge to use the AED, It will be futile.

You should ensure an effective AED programme in place to train and refresh skills once a while, that includes:

AED should be made available in 1.5 minutes brisk-walking distance
AED to be placed at a prominent location with clear signage
Identify the first-aid response team, and ensure they are regularly trained and confident in applying CPR AED

No one can predict nor fully prevent a heart attack or a sudden cardiac arrest. But once you learn to differentiate them and recognise the signs, you could get critical first-aid, for yourself or others, without delay.

When sudden cardiac arrest (SCA) strikes, it is often deadly. It strikes without warning in the most unexpected situations, regardless of your age, gender and lifestyle.

An employee of Royal Plaza on Scotts (who wished not to be disclosed) was grateful to his fellow colleagues, who saved him when he collapsed with an effective combination of HeartSine Automated External Defibrillator (AED) and Cardiopulmonary Resuscitation (CPR).

On 25th November 2018, the employee was found lying motionless in the back office at 07.51am with no signs of breathing and heartbeat. The security team was immediately alerted of the incident and dispatched one of the first aider to the scene. Witnessing that it is a sudden cardiac arrest, the first aider quickly called for an AED from the Fire Command Centre and performed CPR instantly prior to the arrival of AED.

When done correctly, CPR prevents irreversible brain damage by applying chest compressions at the depth of 4 – 6cm and a rate of 100/min in order to pump oxygenated blood supply to the vital organs.
A victim’s chance of survival can increase from 6% to 74% if proper CPR and AED defibrillation is administered within the first 4 – 6 minutes of collapse. Without it, the chance of survival of the victim will drop by 7%–10% every minute.

Fortunately, the first aider acquired the AED within minutes, apply CPR continuously and delivered a total of 5 shocks before the paramedics took over. At 08.27am, the employee was rushed to the Tan Tock Seng Hospital.

On 6th December 2018, the employee was discharged from hospital after recovering from the traumatic event.

Forward Hearts – A Gift from The Heart

He suffered a sudden cardiac arrest and was fortunate to have an automated external defibrillator (AED) to his rescue.

As part of an initiative by manufacturer HeartSine called “Forward Hearts”, which allows survivors of sudden cardiac arrest who were saved using HeartSine AEDs, to donate an AED to any organisation of their choice.

The survivor has decided to donate a HeartSine AED to Singapore Cancer Society in hope that an AED will be available to help save someone’s life during sudden cardiac arrest emergencies.
